Glaucoma
HIV or AIDS
Parkinson's Disease
Multiple Sclerosis
Epilepsy
Cachexia
Cancer
Terminal Illness Requiring End-Of-Life Care

The Process:

Make an appointment by calling us at (203).869.2801 or by email info@greenwichmedicalmarijuana.com

Gather any medical records that support your diagnosis of a qualifying debilitating medical condition and fax them to (203).869.2803.

Confirm that we have received your medical records before your scheduled appointment.

On the day of your appointment, arrive at our private medical office in Greenwich, CT at least 15 minutes early to ensure all of your paperwork is complete.

*NOTE: If you would like one of our physicians to determine if your diagnosis is consistent with a debilitating medical condition, they will need to review your background information, obtain a medical history, review any previous laboratory studies, diagnostic imaging and/or other testing results as well as conduct a physical examination.
